Rheumatic fever symptoms, Peripheral vascular disease, and systemic embolization.

Congenital history of prenatal, natal, and postnatal conditions. Clinical examination of general symptoms and signs, local cardiac symptoms and signs by inspection, palpation, and the classic cardiac method of assessment; auscultation.

            

            

Electrophysiology Study

One of the highly specialized cardiology module sheets; EPS sheet is designed to help cardiologists studying electrical physiology of the heart. General history and examination including history of cardiac medications use. ECG findings during rest with localization of accessory pathways.ECG during tachycardia differentiating between narrow complex and wide complex tachycardia. Other studies; Echocardiography, Halter monitoring, Coronary angiography, Exercise Test, and Tilt Table Test.

EPS study findings include basic study findings, programmed stimulation results, and tachycardia findings. Radiofrequency ablation includes target, site, and results. Finally; complications resulting are identified into 3 main categories; Vascular, Arrhythmias, Ablation complications.

            

            

            

Adult Cardiac Catheter Registry

This clinical sheet is designed to register different catheterization data resulting during this highly skillful procedure. Admission data are categorized into general data of indications, risk factors, and cardiac history.

Catheter data includes coronary anatomy, graft lesions, and specific coronary lesions. Detailed data capturing of up to five different coronary lesions and up to three different graft lesion. Registry of Inlab and Out of lab complications is provided. A final complications management planning is available.

            

            

Pediatric Cardiac Catheter Registry

This clinical sheet is designed to register different catheterization data resulting during this highly skillful procedure performed to neonates and children. In the basic data section; anesthesia and pre procedural medications are identified. Vascular access of venous and arterial catheters is defined.

Catheter course is mapped to include up to three different catheters used at the same session. Measurements section helps automatic calculation of Oxygen saturation and content at different vascular points, pulmonary blood flow, and systemic blood flow with variable shunt levels. These measurements are captured before and after I.V. medications use.

Further calculations of pressures at different points, pulmonary blood flow, pulmonary vascular resistance, and systemic flow and resistance is dependent on the shunt level. Diagnosis of cyanotic and non cyanotic heart diseases. Data of interventions and different complications categories is added.
